Adoption Agreement
Adoption Agreement *Please provide the animals name and ID number. (Ex; Bella, D25-01001)
1) I am aware that sheltered animals may have unknown medical, behavioral, and/or genetic history. Knowing this, I understand that the animal may become sick, and/or change behavior after adoption. 2) I understand that shelter pets tend to be more unsure of new owners/families and surroundings. I understand that my pet may take awhile to “warm up” to me and/or my family. 3) I understand that if my application is approved and I adopt an animal, I am then responsible for any and all expenses incurred. These expenses include but are not limited to: veterinary, grooming, basic care, training, boarding, etc. 4) I understand that if my application is approved and I adopt an animal, I will not abandon, neglect, abuse, or surrender my pet.
